Help with a sentence

Hi everyone,
I'm after a little bit of help. I've been struggling with the following sentence .
"My heart goes out to you and family"
Can I use it in a supporting kind of way or is it used when things have already turned bad?
Any suggestions would be appreciated.

Mon dictionnaire, Collins Senior, dit :
"My heart went out to him in his sorrow -> J'ai été de tout cœur avec lui dans son chagrin"
